Yes. I kept falling asleep during the test and breathing 'wrong' when answering, which throws the calibration for the machine off, but after about a half an hour I passed. I felt bad for the guy taking the test.
In the end, the tester said I wasn't very good at lying, which I wanted to contest, but it seemed counter-productive at the time.
Different organizations have different testing criteria, and some go more in-depth than others, depending on the level of clearance you're going for, or whether it's for a commercial job.
Don't lie to them and you'll be okay.

Yes. I had to take one for a job interview but they never told me how I did. It's very nerve racking to take one. No matter how honest you are, all the stuff hooked up to you makes you feel like a liar. You become very aware of every breath you take. I'm not really sure how reliable they are. It all depends on the experience of the person giving them.

No, it sounds interesting, though. I think I'd flunk even if I didn't have anything to hide, any kind of pressure makes me nervy.
