The media started doing this early in the bush presidency. They decided at some point that if they don't like the president they will show him disrespect by saying Mr. instead of Mr. President or President.

When he was alive he was called Mr. President, or President Ford, I never heard him called Mr. Ford after he became one of our Presidents. I feel that the title of President should always be used to honor any President or former President.
