Elbow grease is an old term for working hard at something. My mom used to tell me, "put some elbowgrease into that" when I just sort of cleaned the dishes. I think it started out as a joke in the factories for someone to say about a lazy co-worker: "Go fetch some elbow grease from him".

Elbow grease is a slang expression for you doing the work with your arm. If something requires elbow grease it means you will need to do some work, usually scrubbing back and forth.
