I hate people who promise to do things, and then don't do it. Even if it's just saying they'll stop by, or we'll get together later on. Or my favorite, a boyfriend who for my birthday gave me a card and said we'd go away somewhere for the weekend. Well, since I wasn't planning it, it never happened.
Say what you mean and mean what you say. I can't stand BS or someone saying something because they think it's what I want to hear.

My pet peeve is vocal music/musical theatre majors who smoke! Why on earth would you destroy your voice when you are planning on using it to make a living?
Other people's pet peeve that I can't understand is hating the sound of water bottles falling on the floor. I know people who simply can't stand that sound, and hate it when people drop their water bottles.

Interrupting when others are talking. Respect a person enough to let them finish what they are saying. Give and take, it's a conversation. Also, how can people leave dishes in the sink for more than a day... nasty.

People who don't put down the divider on the conveyor belt at the grocery store after they put their grocery's down. This really annoys me.

My friend always has to match, and I can't match. It drives her crazy! And my other friend is constantly cracking his knuckles. One of the first things I ever told him was to quit cracking his knuckles!
