I have the same problem. First....try and lose weight and watch your posture. Try and stand straight....I wear a back brace and it helps a great deal.
Second, swimming is great also. I do water exercise because it takes the stress off my back (compression fractured 6 vertabrae in my lower and middle back and tore the rotator cuff in my left hip)
GOOD shoes with good support will help ease the pain also.
Also, when its really bad, lay on your back on a hard surface and put a pillow under your knees. It relieves the pressure.

See your chiropractor more often. Explain your discomfort and follow his/her directions. Most Chiropractor offices also have a therapy.,rehab specialist. They work with stretching and strengthening the problem areas.
For most back discomfort stretching works well. When you get up in the morning do slow gentle stretches right away. Getting one of those big exercise balls and slowly laying and rolling it up your back is good.
But with the history you described I think it better that you consult your doctor and/or orthopedic doctor.

Your chiropracter should be giving you exercises! Or go to a physical therapist. Any exercises that strengthen the back or abdominals can help, but depending on your injuries, some may be contraindicated.

I think stretchs would be the best for you inside of a hardcore workout. That might make it worse. Try stretching every which way you can think.
