It depends.
Depends on the show, the dancer, the theater, the budget, etc.
If a show is being built from scratch, every dancer will have a costume custom made for them. Broadway shows with multiple understudies will have a separate costume for each understudy.
Costumes can be built in-house, but are sometimes hired out to big shops. Most shows employ a bunch of warddrobe people backstage to clean and mend any rips and tears.
Revival shows, or shows without extravagant budgets will either rent and alter costumes or pull from their stock (costumes kept from past shows) and refresh the look for the current show.
Generally, it is not up to the dancer to provide their own costume.

someone would give them each time they star in a show. but i guess the shows would be by different companies, so they would maybe lend, maybe give, maybe remake the dancers new costumes, or simply reuse the costumes used the year before by either the same or retired or fired dancer. someone employed by the theatre woud make the costumes. usually they would make new costumes, after taking body measurements.

depends on the show or the dancer. Lot's of minor roles have costumes straight from wardrobe, lead dancers usually have costumes made for each show's run. Friend of a friend makes costumes for the Royal Ballet - ten years ago she made a costume for Wayne Sleep from fabric with hand sewn faux pearls and glass beads on, the material was specially made for her to use - and cost £80 a metre!
He only got the one costume - well made to allow movement - with a seamstress backstage for quick repairs!

Costumes are reused to keep costs down. For example the same costume will be worn for the same number repeatedly.

I guess they all come from the costume dept. They will probably have a lady there who makes and alters them
