My gosh, I have the same exact problem, I guess I'm not alone. I know how it feels, right now I'm sitting in front of the computer, back not even straight, and when I try I lose patience and sit crouched again. What I'm doing these past few weeks is sleeping straight without a pillow behind my head, looking straight to the ceiling. I used to have a similar problem, make sure that you are trying to straighten the right spot in your back. Sometimes the problem spot is not actually where the wrong posture is more visible (Mine was under my neck, and I actually fixed it keeping my lower back straight). If not, you might wanna go see a doctor. Good luck!

A tip I like to give my patients to think about having a string going from the middle of your head up to the ceiling. It is gently pulling up and as you imagine that, you will feel your spine realign . ears over shoulders, shoulders over hips......in standing.
Pull your abs in as well.
It will feel odd so I suggest:
Work on this gradually. Be conscious of this with tasks like standing at the bathroom or kitchen sink, standing in line at the grocery store, while seated at your desk or working and gradually develop a habit.
Work on abdominal strengthening through pelvic tilts and crunches/reverse crunches, work on strengthening your back to combat the forward curled shoulders, and stand tall and proud, no matter how short you are. :)
If you are seated a lot, esp. in front of a computer, change position frequently and at least hourly, get up from your desk and move around. This will help prevent that forward hunched position ( with the chin jut) so many of us adopt sitting in front of a PC for hours!

if you're really that serious about it, you could use packaging tape and wrap it around your chest while sitting straight, that way you can't bend/slouch.
when you sit down to eat, you can tie a cloth across your shoulders and tie it off in the back so that way you are forced to sit up straight( like in the movie Princess Diaries)

oh, god, how i love answering this question. i have the same kind of problem as you. well, what my mom does is she kind of pokes me on my back, like she pokes me that you sort of notice it..and that usually works for me in the very short-term..but you can't have your mother with you all the time..lolz she just reminds me of it when she sees me..but i guess just pretend there is someone or something poking you in the back or poke yourself.. i know that sounds cheesy and corny but i sometiemes do it, and then i straighten up. just remember how you look when you slouch, you look old..haha. .lol. oh, another trick is, this is a very good trick i think. put a book on top of your head (not too heavy) and then try as you sit and walk try not to let it fall of your head. or maybe just a heavy book and have someone fasten on your head lightly with tape or something else..lol i dunno but i think the book thing is the best for you. I grew up with girls like this. Their mothers sent them to ballet and modelling classes. After taking it for a while, it became natural for them to sit and walk with excellent posture.
