the height of a punching bag depends on your height and what you are trying to acheive with it, but normally you want the top of the bag to be right above your head, depending also on the size of the bag and weight

Your eyes should be even with the bottom of the bag if you are referring to the speed bag.

You should periodically adjust it so all the pounding doesn't take place in one section.
