I think a lot of it is learned at a very young age....when we first run into proof of the proposition that "Life is Unfair", when bad things happen (often to us ourselves) for little to no reason, in *spite of* being good and not doing anything wrong, well....
It's our first "sane response to an insane situation". We learn to blame ourselves because some of us in that young, innocent state reason from effect to cause ("bad things happened, so I must be a bad person") instead of cause to effect ("who or what really made the bad things happen?"). It helps us cope early on, because the alternatives (blaming our parents, screaming at grown-ups twice our size, telling our teachers *that they lie* to their faces) range from the useless to the suicidal (depending on how hostile, abusive, etc, the adult in question is).
And then.....you run into those who clearly have *benefitted* from the Unfairness, and who have an agenda and *want you* to blame yourself for your problems, because that makes it easier for *them* to manage and control *you*. Truth is, a lot of what makes the *working* world go around, what keeps the rich people rich and the poor people poor, is *about* keeping folks guilty when they don't need to be, keeping people blaming themselves so those who Reap the Spoils of Unfairness get away with whatever, whenever. If someone constantly blames himself in the name of a politics of "personal responsibility", they won't ask for help until they've lost limbs, are nearly dead in a gutter, and so on....it's basically about "working a horse" until the "owners" want the horse shot.
So yeah...it's a power issue basically. People know life is Not Fair in the slightest, but by the time many figure it out, there's already too much time and effort wasted *supporting a system that screws them*.
So rather than lose it, risk everything and rage against Those Who Have....they *keep on* blaming themselves for things *clearly* not in their control and spiral further downward into despair and suicide. Which suits those who Reap the Spoils *just fine, thank you*. Why shoot "the horse" when you can trick your Plantation Labor into killing itself for you?
And speaking of Dead Horses....I should stop now. ;)

Some times I think its hereditary. A lot of it is learned in the first five years of life.
There are some who have no guilt. That is bad.
