Probably not. The only thing that is possibly fatal and related to tic bites is Lyme Disease. If caught early, it can be treated and not be a problem at all. You could have lyme disease if the area around the tic bite becomes inflamed and looks like a bullseye--but if that doesn't happen, no worries. Even if you got Lyme Disease, many people live full lives with it.

Theoretically, yes. If the tick goes unnoticed, you can die from the toxin build-up (I think it eventually leads to heart failure, but don't quote me on this, I'm not a doctor). Fortunately, most competent doctors know the symptoms and can find the tick before it reaches this point.
You could theoretically also die very quickly if you were allergic to the tick, although I think this is fairly rare. People can be allergic to almost anything, though.

Yes and No. u can die if u have bad allergies, and even if u don't, go to the doctor to remove it. Or else if u do not and take it out urself, the head might still be there and it'll suck ur blood!!

Ticks can cause rocky mountain spotted fever and lyme disease both serious illnesses but I have not heard of a tick that kills anyone.

you have to get it looked at. if you can take the tic out, you shld be ok. i think you have to get it checked out and they can give you medication for it.
