The DVD ends differently than the U.S. theatrical version. The DVD has the UK theatrical ending. Both endings were good, and completely different.

If you watch the interview with the director in the DVD extras, he explains both the short and longer endings. The woman at the end of the movie lost her mind from all that she experienced. She never left the cave, but was able to escape in her mind to a better place.

This leaves you to imagine, whether she lives, or dies, becomes one of them etc.

Not sure if I understood the ending. I just watched the DVD. Kelly, the girl who lost her daughter wakes up all bloody after having a dream that she killed Juno and escaped. She's still in the cave and has that vision of her daughter and the birthday cake. I really don't understand the allagory here. She slowly lifts her upper body and makes a gesture like she's reaching for the cake with a smile. So did she finally snap and lose it mentally? My fiance's reaction was, "Oh she became one of them..." I'm not too clear on it, but that's what happened.
