Yes, I am always convinced that I am not going to survive it.
I think for one thing it's being trapped inside this limited space, but I am also really terrified when the weather is bad and it gets really bumpy.
I always take a tranqullizer about 1 hour before take-off, so by the time the plane gets up into the air, I am kind of calm.
When the weather is nice and it is not bumpy, I sometimes even enjoy flying.

yes, oh yes. I flew with my daughter to Connecticut a couple years ago and I was petrified. It is claustrophobic for one thing, I am a tall person, secondly it is very cramped and uncomfortable. I guess after 911, more people are scared now than ever. The seat belt light was on most of the way, it was bumpy and I felt trapped. It was the longest two hours of my life and I was glad to be on the ground. I would only fly in an emergency from now on, I like driving and seeing what is out there even if it takes longer and is more tiring.

no honestly I love flying, I fly constantly (once or twice a week usually) and I love it. It is driving that I am deathly afraid of (not literally but I don't like it either, I live in a city and don't own a car). Flying is far safer statistically than driving.

Anytime I have to travel by plane, I have my doctor prescribe a sedative. It really helps keep me calm.
