Wow! That's a tough one.. lol.. I'll try my best.
First of all is your friend marry? If so then maybe BOTH parent can sit down and talk to their daughter explaining that they love each other ssssooo much and out of that love they had HER and every since then they have enjoy being her parents that they wanted to continue that love with another child so the family will grow.
O.k. now that was a sugarcoated talk but it might work?
However if your friend is a single mom then she might carefully say something like that she had enjoy her little girl sssooo much that she went to the doctor and see if she can have another one. The doctor said yes but that the mommy won't know want it is a brother or a sister until later on and if that was o.k. with her. And now there will be three of them in the family.
Well, this is the best I can do without knowing a whole about your friend but I do wish both of you luck and hope that you will be there to help a little with "The Talk" in case if she gets stuck.
:-)

Good day and Happy New Year.
Love and Peace

I don't know that she should be necessarily told about sex. But it would not hurt her to know that there is a special place the the baby would be born thru that only mommies (or girls) have. Too much info would probably scare her. It will not hurt her to know that the parents were given the baby because they love each other very much. If they are a Christian family, explain that God choose to give them the new baby because they love each other so much or because she needed to be or would be a a great big sister.

I personally think that four is too young! That's just my two cents worth. My daughter is eight and she thinks they come from the mom's belly. She came up with that on her own. I think that by telling them where babies actually come from, there would be too much other information that they are just simply too young to know!
