As-salaamu'alaykum

The Shahadah is as follows

Ash-hadu Anla ilaalah illillaah Wa Ash-hadu ana Muhammadan 'abduhu Wa-Rasooluh (Rasoolillaah)

Which means, I bear witness that no diety has the rigth to be worshipped except Allaaha and I bear witness that Muhammad is his slave and Messenger

And the Translation of the Salaah, it's good to hear what you said, about not saying a prayer without knowing the meaning, that's good brother! I learnt it myself but forgot parts of it. And am trying to learn it again.
And I would be more than happy to help you

So heres the best thing you can get on the translation of Salaah

http://www.qss.org/articles/salah/toc.html

It's a really good book.

Also please do contact me on epyon000@hotmail.co.uk and I will teach you more about Islaam, Insha'Allaah

Was-salaamu'alaykum

Shahada:
أشهد أن لا إله إلاَّ الله وأشهد أن محمد الرسول الله
'ašhadu 'al-lā ilāha illā-llāh, wa 'ašhadu 'anna muħammadan rasūlu-llāh
I testify that there is no god (ilah) but God (Allah), and I testify that Muhammad is the messenger of God.

Salat: This seems to be more complicated. Here's a website that's very thorough about what to say and do: http://www.islamicacademy.org/html/Learn_Now/English/Salah_In_Audio.htm . The instructions are in English, but the prayer is in tranliterated Arabic. This (http://www.islamicacademy.org/html/Learn_Now/English/salah_translation.htm ) is the related site that has the English tranlsation of the prayer.

I would recommend u to go to a nearby mosque. Usually, they'll have class for the new members.

I means, even if i give u the Arabic, can u still pronounce it correctly? It is much difficult to learn via books or websites than learning from people who are experts in this field.

So just go to your local mosque, if u can find the imam(leader) of the mosque, just tell him that u are a new convert (mualaf) and see if he can help u. I'm sure u can find help there.
May God bless u.

I'm Arabic, and I'm good at English. You can send me an email to tell me in Arabic what are you looking for, because I couldn't understand well

Definitely don't go in a Catholic Church then. I went to one service and half of it was quoting from a Latin book that was not translated.

I learned Greek though so I can read the New Testament in its original language. Totally cool.
