An explosion causes a shock wave and if it is big enough, and you are standing close enough it will kill you simply because of the pressure of the blast. There are things called blast bombs which are designed to spread the blast of an explosion over a wider area, and are specifically designed to kill people whilst leaving building relatively undamaged.
As others have said if you are talking about grenades, they have to have some casing, and it is this which causes the shrapnel that kills. Fragmentation grenades are designed to fragment upon explosion and release as much shrapnel as possible hence the 'pineapple' design. The explosion splinters the little squares and they fly out in all directions at hundreds of miles per hour. if they come into contact with human flesh its not a pretty sight.

Shrapnel is the little bits of metal that explode off of a grenade. Grenades are shaped like pineapples, with the little squares molded into the surface to maximize this effect. Shrapnel also consists of flying debris from soil, like rock and sand, which can perforate living tissues as well.

Well I am a U.S. Marine and this commen knowledge in the Corps. If the grenade is within 5 meters, yes.

Grenade has layers. From inter to outer: firing mechanism, powder/explosive, shot (round balls of steel), the shell. It's the shot and bits of the shell that's suppose to do the killing.

Loss of blood by mass penetration of shrapnel along with shock .
If they are lucky to be knocked unconscious by the blast( no pain)
If conscious the pain may be nill as the body at first would nullify the trauma but shock would kick in and the pain would be horrendous then loss of blood or shock would kill.

1) the shrapnel from the hand grenade
2) the sudden impact of higher air pressure which can cause internal damage to the human body.

I once watched a war film as a kid and asked this.
My Dad blew in my face really hard and then said:
"Imagine that, but a billion times harder".
Followed by small bits of metal hitting you at the same speed.
