one relaxer that is great but may not be available at sallys is the paul brown system. it has sooo many vitamins and nourishing agents. i'd suggest you call around to see who has it.

Lady Velvet or General Treatment. If applied right your perm should last 10-12 weeks at the least 8 anything over that is damaging so it is better to seek expert help.

Depends on your hair type. I get Just For Me and it lasts about 3 - 4 months.
