On a full-suspension bike (especially downhill bikes), the action of the suspension, the jarring of rough terrain, and the relative weakness of the springs in the rear derrailleur makes keeping chain tension a challenge so a chain tensioner is used to help prevent throwing the chain off the drivetrain.

Can also be used to keep chain tension on a single-speed bike but the horizontal dropouts that holds the rear wheel can be adjusted to take up chain slack.

To take out any excessive slop in you chain without stressing it. Reduces the chances of the chain coming off.

In most cases, they are used to take up slack on a chain on a single speed so it won't fall off.
