It could be the smell is emanating from your insides, not your mouth. Sometimes people have digestive problems that contribute to their breath being bad. It could also be your gums, as you asked. Also, do you brush your tongue as well as your teeth? That goes a long way to making your breath fresh. And do you floss? Do you have any cavities? These are just some suggestions, but ultimately you're going to have to check with your dentist.

More than 90 percent of dentists say they regularly see patients with chronic bad breath or halitosis. But while bad breath is one of the more embarrassing problems, it can be one of the easiest to solve.
Most cases are caused by bacteria in the mouth or on the tongue. Regular brushing and cleanings are often all the treatment needed to freshen your breath. But bad breath that doesn't go away may be a tip-off to a more serious medical condition, such as gum disease, diabetes, kidney disease, or liver failure.
If you just can't freshen your breath, see your dentist for a thorough exam to rule out tooth decay or gum disease. Then brush, floss, and use a tongue scraper every evening to remove the bacteria from the back of your mouth.

Most of the bad breath issues are created from your stomache, not your mouth. However, it wouldnt hurt to go to the dentist because an absessed tooth will also give you a foul smelling breath. Check with your dentist first and if your teeth are all good, its your stomache. Some people just have to suck on mints or chew gum all their life. But, there might be some stuff you can get for stomaches. Check into it.

Yes, it could be cavities, gum disease, or even a gum infection. Your best bet is to go and see a dentist. He can either help you or lead you in the right direction. Best of luck to you.

Do you floss...If you floss everyday, you should be good. If you haven't flossed in a while, you might bleed somewhat...but that will stop after you get used to it.
