If you're referring to torsion of the testicle, you'll know. He'll be in constant pain and discomfort. When I was little (about 10 or 11) that happened to me, I thought I had a hernia or something because I didn't know anything. I went to the hospital and the surgeon told me I should get surgery immediately. So, I did. Everything still works too, I have a beautiful, healthy daughter, I don't have any problems urinating or anything, but I do feel like they're a little more sensitive than other guys' nads. (They sew them to the scrotum wall to prevent repeated twisting). So it's not that the actual testicles twist, it's the tubes attached to them.
So yeah, that's the story about my balls.
If you think the kid has torsion of the testicles, please, for him, get him to the hospital to have it checked out.

Serious pain/serious problem. There will be lots of pain, and get to the doctor fast, because a twisted nutt over a short period of time can kill the testicle and its nerves.

Yes, take the child to a paediatrician. Don't make guesses, or take risks when it's a matter of a child's health and welfare. If you are noticing an appearance that doesn't look right, that would make you suspect something like this, you are going to have to get it confirmed anyway. Better to do that and be thankfully wrong, than do nothing and create an even worse problem.

Trust me if its twisted youll know, hell be in a whole lot of pain and youll actually probably have to take him to the emergency room to get it "untwisted".

I had a twisted teste when I was a young lad. It hurt. BAD. I doubt that you can really tell...unless you get complained too.
