Spanish: "Mi corazón simpre va contigo" or "Mi corazón siempre está contigo", or if you say in a very informal way: "Siempre te llevo en mi corazón", which is "My heart is always with you."

I'd say "Te llevo siempre en mi corazón". You can find other sentences, maybe more elaborate, but this is the most natural way to say it
