Condoms can be ineffective for any of the reasons you mention. There is no guarentee that ANY birth control method is fool proof. Only abstanance is guarenteed not to make you pregnant. I'm not saying that is the practical answer, just the only one that works. Aside from that, you take the chance of an unwanted pregnancy.

It only happened to me once and it was a SICO ultra sensitive condom. We weren´t being rough, I just think that the extra/ultra sensitive ones can be made too thin and thus easier to break. I ended up taking the morning after pill.

First and last time condom broke... i was 16 and lets just say there wasnt enough lubrication which caused the condom to break. My course of action was, first telling my mom what happened, then went to the OBGyn to get checked out and got right on birth control.

Condoms are no longer ninety 9% effective. they ruin oftentimes while there is too lots friction or no longer sufficient lube. in basic terms lube up and understand that condoms are not all that risk-free. My reports are that a million of 5 ruin.
