Feet on the table are not a good idea in a restaurant, for anyone. It's just too casual for those kind of situations, and dirt can fall off of one's feet or shoes and get into the food.

Not okay.
Although I personally don't find it as gross as putting bare feet on a table, shoes still have germs on them that you wouldn't want on your eating surface.
Also, there's a potential that the shoes could damage the table.

Yes, if she's the queen or something i dont think you should tell her to get her feet off
however, if she's anyother ordinary woman, she should be considerate and take off her feet from the table. Imagine eating and smelling those feet.

I don't think so, but it is ok for someone without hands to eat with their feet without there shoes. Since some people don't have arms.

Reasons of health and hygiene.
Basic rule of etiquette deems that feet do not get plonked on restaurant tables.
However, private dinner parties have a different code of etiquette altogether.

No. It is not her furniture and shoes can scuff things. Furthermore, it is rude to put dirty things on the table around people's food or in their faces.
