Start with a professional cleaning and evaluation in your dental office. After that, you have to maintain your clean mouth at home.
Floss, brush your teeth, scrape your tongue, rinse with a mouthwash of your choice.
Avoid foods that make bad breath, like onions and garlic. If none of that helps, consult your doctor or dentist to see where the smell is coming from.If you do a search on the internet using "bad breath" as your search term, you will find almost five MILLION references. Here's a good place to start:

The odors from garlic and onions enter your blood stream and then it takes a couple of DAYS before you breathe it all off. As you do this, you will continue to have bad breath. You can only use a mouthwash or breath mint (sugarless) as needed to mask the odor. I have read a lot of answers here saying to eat parsley to get rid of garlic breath, but I have no personal experience with this. Try it.

hi Persistent bad breath or a bad taste in your mouth may be warning signs of gum (periodontal) disease. Gum disease is caused by the buildup of plaque on teeth. The bacteria cause toxins to form in the mouth, which irritate the gums. If gum disease continues untreated, it can damage the gums and jawbone. Other dental causes of bad breath include poorly fitting dental appliances, yeast infections of the mouth, and dental caries. try this steps Instructions Things You'll Need: * Toothbrush * Toothpaste * Dental floss * Tongue scraper * Sugar-free mints or gum Step 1 See a dentist. Chances are you suffer from the typical bad breath moments that all people experience. However, there is a possibility that your unappealing breath is something more serious. Having a professional opinion helps you rule this out. Step 2 Kick the morning breath blues! The morning is the best time to get rid of bad breath stored up over night. Brush and scrape as soon as you rise and no skipping on the weekends! Step 3 Brush your tongue! Hundreds of germs live on the tongue and the strongest muscle in the body is a major contributor of bad breath. Tongue scrapers are very cheap and a great investment. If you don’t have a scraper, use the toothbrush by scraping lengthwise, from the back of your mouth forward. Or if you’re really desperate, a clean credit card is a great scraper substitute. Step 4 Floss! Try to floss once a day, every other day if your teeth and gums are sensitive. There are many options when it comes to flossing. Floss comes in many forms. Pick the type that is most comfortable and easiest to use. Step 5 Hydrate! In between meals and while you sleep, your mouth’s saliva levels may fluctuate. A medical condition called xerostomia, better known as dry mouth, can occur when you are lacking fluids. Saliva is essential to wash away those leftovers from lunch. Step 6 Keep sugar free mints and gum handy for quick fix moments. Used sparingly, mints and gum can offer immediate bad breath relief when you’re in a pinch.

Main sources of bad breath:
1) plaque
2) dry mouth
3) acid reflux from stomach
4) tartar buildup on teeth, active gum disease, or gross tooth decay
5) malodorous food choices
Main solutions for bad breath
1) brush (I like electric), floss, tongue scraper (I like Discus Dental's), then Listerine rinse (full strength, full 30 seconds) twice a day
2) drink lots of fluids. Review your prescription medications with your doctor; many prescriptions can cause dry mouth.
3) Talk to your medical doctor about medications to prevent your stomach contents from traveling up your esophagus. Not only can that stink, it can damage the lining of your esophagus.
4) get regular dental cleanings at least every 6 months. If a tooth has a problem, treat it promptly before it becomes a bigger, more expensive, and potentially more smelly problem.
5) hold the onions and garlic, please
Treating bad breath is at best an inexact science, so YMMV on any or all of these suggestions. Most of them are easy, and won't cost you much. Finally, beware of people offering quick, universal solutions for $19.95. We all have slightly different causes for our breath, so we'll all have slightly different solutions, too.
