Start with a professional cleaning and evaluation in your dental office. After that, you have to maintain your clean mouth at home.
Floss, brush your teeth, scrape your tongue, rinse with a mouthwash of your choice.
Avoid foods that make bad breath, like onions and garlic. If none of that helps, consult your doctor or dentist to see where the smell is coming from.If you do a search on the internet using "bad breath" as your search term, you will find almost five MILLION references. Here's a good place to start:

The odors from garlic and onions enter your blood stream and then it takes a couple of DAYS before you breathe it all off. As you do this, you will continue to have bad breath. You can only use a mouthwash or breath mint (sugarless) as needed to mask the odor. I have read a lot of answers here saying to eat parsley to get rid of garlic breath, but I have no personal experience with this. Try it.

Brushing is very important but flossing is just as important.
If you don't floss food particles will get in between your teeth
and cause bacteria which causes bad breath. Floss your teeth
every day then brush. After that use a antiseptic mouth wash.
And don't forget to brush your tongue. After the first time you will
notice fresher breath. And steer clear of breath mints they wont
help, they can actually make it worse. Also see a dentist because I believe having cavities can also cause bad breath.
Good Luck. And remember it's all about the floss'

brush your teeth in the morning and after you eat and when you go to bed.
use mouthwash at least once a day
FLOSS!!!! it's amazing how much nasty smelling stuff comes out when you floss your teeth.
brush your tongue and your gums and the roof of your mouth
chew sugar free gum and suck on sugar free mints
DRINK LOTS OF WATER!
avoid cigarrettes and alcohol and things of the like (not only are they disgusting and unattractive, they make the breath smell horrible)
avoid too much coffee, garlic, and onions
if you haven't visited the dentist lately, you may want to consider that if you are able to see one.
if doing the things above does not help ...
are you taking any medicine? perhaps this is a side effect
do you have sores in your mouth and/or throat?
i would strongly suggest you see your doctor and/or dentist and discuss your problem with them because bad breath really stinks, and i totally feel for you!

Bad breath is caused by bacteria. Brushing only gets rid of a small amount of bacteria.
Brush after every meal and use Listerine, they have many good flavors now.
See a dentist every 6 months, they can clean below the gum line where most of the bacteria grows and cannot be reached by brushing.
Do these things and you will not have to worry about your breath. I also carry breath mints if I know I will be in close confines.

Do you just brush your teeth and gums? Add brushing your tongue and the roof of your mouth.. If this goes not help. Then you may have a bad digestive system. Also what you eat such as spicy foods will not only affect your breath but also your body odor.

Sounds as if you have bacteria in your mouth, this could be on your tongue, gums or cavities. Please see a dentist immediately before the problem gets out of hand. By the way have you test your ton-sills? tonsillitis do give you chronic bad breath until remove.
One other thing to know is that most bacteria is cause by cavity(ties).

If it is a chronic problem, see your doctor first, then your dentist. It could be a medical problem (such as post nasal drip), or you may have rotting under a filing or a absess in your mouth.
Also try taking some chlorophyl (from the health food store)

make sure you floss and brush more frequently through out the day, get one of those new mouth brushes and "brush" your tongue and the inside of your cheeks...
rinse with some medicated mouth wash and/or some listerine thin strips
check with your dentist to see if you have gingivitis or another mouth ailment

It could be something you eat/drink or drainage. Try a mouth wash or brush more often. Or you could always chew gum or a mint.
