If you're speaking to someone older than you, you want to give them respect so you say: Como se llama.
If you're speaking to a friend or someone younger than you, it is casual so you say: Como te llamas. (the 'll' are pronouned like a y).
Then "how old are you" is the same way.
Respect form: Cuantos anos (with a tilde over the n, otherwise you'd be saying something inappropriate) tiene usted.
Casual form: Cuantos anos (with a tilde over the n again) tienes tu.
