1. Depressed mood most of the day, nearly every day.
2. Diminished interest or pleasure in all, or almost all, activities most of the day, nearly every day.
3. Significant weight loss or weight gain.
4. Insomnia or hypersomnia.
5. Fatigue or loss of energy nearly every day.
6. Feelings of worthlessness or guilt nearly every day.
7. Lack of concentration.
Physical pain is involved and it is not always due to self inflicted injuries. Being depressed for days at a time can cause aches, pains, shortness of breath, etc.
The mind is very powerful.

there is a wide range of symptoms and levels of bi polar disorder.
It is also one of the most commonly misdiagnosed mental illness right now. As far as physical pain. I'm not aware of any. However suicide self inflicted pain and dangerous activity is highly likely the longer it is left untreated. I personally would rather have a broken arm every few months than some of the symptoms that are associated with this mental illness.

For me, the depression is the most debilitating part of it. Bipolar depression tends to be intense. I have chronic episodes of it and have all my life and at times it leaves me almost completely unable to function.
Physical pain can be a symptom of depression.
Not all people who are bipolar have hallucinations. There are several types of bipolar. I think it's only with Bipolar I mania that can (but does not necessarily) result in hallucinations.
A lot of people, while manic, do things like get themselves horribly in debt...spending money they can not afford to spend.

theres alot more. although i don't have bi-polar disorder, i have borderline personality disorder. those too disorders are common but not the same so i do have some knowledge. bi-polar makes you switch from happy-depressed-angry-manic and that puts alot of work on your body. it can make you really tired, and physically sick. when you are in the happy mode and you just came out of the depressed mode it totally changes your feeling and that can make a person really sick. bi-polar means that you are either on one side or the other.never in the middle it takes energy from you making you lathargic and really tired. bi-polar changes your eating habits from day to day, that can rearrange your body. it also changes your sleeping habits. and your behavior. so depression/anger/excitment/manic anger alters your feelings changes the way you feel physically.

The shattered lives of the people that love them that they leave in their wake. No there is no physical pain. but they often self medicate with drugs( street drugs and prescription pain killers) or alcohol...and won't take their own prescribed medication.
