it would depend on the which style book you're using. there's different styles for different types of paper. you're best bet is to find the style book you need (i.e. Chicago, APA, AP, etc.) and look it up there.

In English writing, most writers use a Style Guide, notably the Modern Language Associations, found at the link below. The New York Times, and even Microsoft have there own style guides. Some of them are for a specific purpose, like technical writing or News reporting. Most of them are based on the recognized definitive work in this area "The Elements of Style" by William Strunk, Jr. properly cited as:

Not all style guides agree, especially about citations.

yes, (Authors Surname page no.) the author's surname will depend on who comes first for the bibliograpy.. or how it is cited in the book.
