If patient not in [illegal immigrant, privileged indigent] then

d(medical patient)/dt = rich doctor + rich drug company + poor patient

d(surgical patient)/dt = rich doctor + rich hospital + poor patient

d(pediatric patient)/dt = rich doctor + poor family of patient

d(OBGYN patient)/dt = rich doctor + poor mom-to-be

However, if patient in [illegal immigrant, privileged indigent], the term on the far right is replaced in every case by "poor taxpayer."
