It's by the mile in most places, except if you're really stuck in traffic and not going anywhere, and then the meter will click over at a certain rate, which is posted.
Where it's metered, it helps the driver to go faster, so he can get more fares in a day.
If you go more than 55 miles an hour, you end up consuming more gas -- it's less efficient.

The fee is by miles/kilometres driven, so for the distance you travel the fee will be the same each time. If the driver is going slower, it's just more time consuming.
As for gas, it depends on the car model, but I think that the two extremes use up the most gas. Going really slow generally uses only a bit less than going fast. Again, depends on the car.

It's the same distance between two points regardless of the speed that the driver is going. If the rate is per meter or mile or kilometer, the end price will be the same rather you go 50 mph or 10.
Typically, less gas is consumed going faster.

around here the cabs charge the same, minimum rate anywhere you go in town--the meter doesn't even get turned on unless you're making multiple trips or leaving the city limits

the meter for how they charge you is connected to the odometer, but it also knows when the vehicle is not moving and will charge for any idle time in traffic, speed does not impact your fare
