Since He can do anything, I think He'll come to many different places at once, probably the places that are causing the most evil in the world, so I guess that means He'll be in every major city in America.

Luke.17
[20] And when he was demanded of the Pharisees, when the kingdom of God should come, he answered them and said, The kingdom of God cometh not with observation:[21] Neither shall they say, Lo here! or, lo there! for, behold, the kingdom of God is within you.
This says the kingdom of God will not come by observation.
Also,
Heb.9
[28] So Christ was once offered to bear the sins of many; and unto them that look for him shall he appear the second time without sin unto salvation.
This says he will appear to those who look for him, so I take it those who are not looking will not see him.
It also says he comes as the thief in the night. The only way you know that a thief has come in the night is either stay awake and wait or wake up to find all is gone. The reference to this is awakeness, it means one must be awake (spiritually not literally) to even know Jesus has come.
Gal.4
[19] My little children, of whom I travail in birth again until Christ be formed in you,
This says he will be formed in you, like a child waiting to be born. Christ grows in you.
So I would say he will make his big appearance to you once he is formed in you. The night sky will them light up like the day time, and for that instant you will be able to see what was once dark. Another way to see this lightning reference, Christ may be formed in you because of a certain area of truth in your life you are seeking, but once he is formed in you, you will be able to see the truth in all aspects of your life.(like the lightning at night, how cool is that).

Good Question, since the earth is round, where does the east start and where does the west end? But since he says it will be like lighting and lighting travels at such high speed, will it really make a difference where it starts, since it will be so fast?

In Israel. When the Jews flee to the mountains, and call up in unison for Jesus to come, He will come.
