It depends on where you are. You must find something that you know about and that she probably knows about (don't ask her if an 86 Mustang is fuel injected or carb'd). If you can find a way to compliment her (without being too cheesy) you'll earn bonus points.
Once you've found the topic, take action! If she makes you feel stupid, you have lost nothing. Just move on to the next one.

Either way a conversation is started. Try at first to stick to yes/no answers. they are safer and less intrusive.

Start out saying how was your day? Start talking about school or something that going around like a sickness or something.

It depends on the situation. If you met her at work, ask her something work related. (Like "I'm new here, can you help me?" or "I'm looking for such and such, can you help me find it?" or if she's carrying a lot of things, hold the door for her (if she needs help) or help her if she drops something.
If at school, find something relatable to both of you. ex.-"Hey, did you hear what such and such did yesterday?"
If its just out and about, you could ask for the time (if you see she has a watch on), ask for directions,comment about what she's wearing, or her smile, compliment her. Hope it helps.
