Always! If you are used to being an honest person, it is very easy to let somebody know you appreciate the gesture, but you would really prefer something else. Make sure you show them what you get instead (or better yet, take them), so they can see what types of things you like. If you hide the truth from them, the chances are high that this will happen again and again. There is absolutely NOTHING unethical or in poor taste about being honest with someone who cares enough to buy you a gift!!

I was once very brutally honest with a certain elder relative of mine who thought it would be cute to give me a matching bra and panties set. I was about thirteen at the time and it was a tender age to begin with. But for that relative to go and announce it to the crowd by giving me what she did to open in front of everyone? It embarrassed me badly and I made it clear to her as to why. She never really did "get it", and to this day I proceed with caution when it comes to opening presents.

People are so frickin' greedy.
Someone tried to think of something that would make you happy, and gave it to you.
You should act appreciative.
Honesty isn't the over-arching virtue it's cracked up to be. Sensitivity to others, and gratitude are more important than strict honesty that has no purpose but to make the well-meaning feel bad.
Unless you KNOW for certain that they gave it to you to p*ss you off. Then a cold "You SHOULDN'T have." would be in order. (They know you're a vegitarian and gave you a ham, for instance.)

When I was seven, I proudly declared that I hated the shoes that I had received from a relative. Needless to say, I didn't get many gifts from them later on in my life.

Yes, and I'm sorry I did it. All you accomplish is hurting someone's feelings. There are some things you should never do and one of them is saying you don't like a gift. Accept all gifts graciously. You can exchange them later.
