I'm a professional translator
the best translation would be:
לזכרך לנצח (in the memory of you forever)
אזכרך לנצח (I shall remember you forever)

If you really want get a tattoo in Hebrew, I strongly suggest to get translation from a professional, otherwise it might just happen to let people read on you a different message from the one you wanted to communicate.

Well, the translation cannot be exact, but here are my suggestions:

לזכרך הנצחי
"le'zichrecha ha'nitzchi"
It is more like: "for your eternal memory", but it sounds good in Hebrew.

לזכרך, לנצח
"le'zichrecha, la'netzach"
More like: "in the memory of you, forever".

While that is EXCELLENT advice simply because of the whole..it will be there forever yada yada yada...which of course you know.. :) but the best online translator I know of is www.dictionary.com. But, since it translates word for word, its very likely to be inaccurate for anything more than a single word because translations are rarely compatible word-for-word when in sentence form.
