a warning would be ears back, tail twitching..I think it wants help..feed it and take it to the vets..most vets will treat strays for free and find them a home if you can't keep it.

It's telling you it is in pain, and if the mewing is directed at a human then yes it is a plea for help, even if it may be skittishly afraid (after all, it's quite possibly been injured by a human).
A cat's warning is very distinctive, with a groaning growly sound, ears back flat against the skull, tail lashing and sometimes fur raised (the "halloween kitty" look).
If at all possible, please take the cat to a no-kill animal shelter. If that's not available to you, take it to the vet and explain the situation -- usually, you'll not be responsible for vet bills in this instance.

It could be both. When I found my cat she was in the rain meowing. She was also injured and it seemed as if she was asking "are you my friend?" or "are you going to hurt me too??" I let her approach me, speaking gently and in a reassuring tone. I would be careful about letting her in your home, as strays often have worms and usually always have fleas.
I ended up taking mine into an animal shelter. She had a badly injured paw. After 4 days she became legal property of the county and was available to adopt. Since I was the one who brought her in, I had first dibs.

Plea for help. You should get the cat to a vet somewhere. It is likely in pain. Just be gentle and move slowly so you don't scare it.

It's frightened and in pain. If you approach it, it will probably attack you.
If you want to help it, contact your humane society and ask if they have a trap you can use (it won't hurt the animal, it's a type of cage) to catch it and bring it to a vet. Sometimes there are local organizations that will do it for you.

First of all, how bad is it hurt? I would never go near a stray because you don't know if it's had it's shots and all. But......on the other hand it might just want help.
It's a gamble, but I would try to help it.

that's sad :(
all i can tell you is that a cat's tail signifies it's mood.
when a tail is up high the cat is happy or content.
when the tail is down, that signifies a lower mood.
when the cat's just walking and the tail is down, that means nothing.
