I think Autism and ADD are often over diagnosed in the US. We have the highest rates than any other developed country in the world.
One theory I have in regards to your question... is perhaps rural mothers, have calmer less stressful lifestyles overall, thus healthier pregnancies.
Whereas mothers in urban/industrialized areas, are often stressed out, working and commuting and doing the 'soccer mom' shuttle etc., so their pregnancies encounter more stress. Thus stress while pregnant maybe a possible cause of autism. Autism affects boys almost exclusively; which, again leads me to the stress while pregnant theory affecting the Y chromosome only.
Just a hunch.
