paranoia is a most often a symptom of a number of illnesses/ conditions.
there is one condition called paranoid delusional thinking.
this is a mental health condition and would be listed in the book called the diagnostic psychiatric manual . or the DSM.
other conditions can result in this sort of thinking. each of them is found in the same book. Schizophrenic persons will at times suffer to some degree this sort of thinking as will others who suffer from delusions . Still another cause of this sort of problem is drug induced . an overdose of some medications as well as the use of a number of different street drugs result to various degrees this problem.
there is one more cause and that is justified paranoia this is very rare and is almost never seen by a doctor but is instead something that would be seen by people who have good reason to think that they are being followed or others have plans to harm them. a person who would have this would have to be in a position like the president or some sort of real secret agent.. since
such people have reason to wonder about such things it is not an illness but it can become overblown within the persons thoughts . some soldiers have been known to return from missions like this but again this is so rare i really shouldn't mention it but i do believe in giving the complete answer if i know it. note post traumatic stress disorder does not cause this thinking

Lots of things can cause paranoia. Schizophrenia and related psychoses, delusional disorder, paranoid personality disorder, severe mood disorders, dementia, delirium, substance intoxications and withdrawals, a number of physical disorders that affect the brain and central nervous system. Paranoia is a very non-specific symptom and you have to look at the whole picture of symptoms before you can even start to make a determination as to what's causing it.

there are lots of mental illnesses that make you really paranoid...such as schizophrenia, the most notorious one, and paranoid personality disorder (yes, there is a such thing), even anxiety disorders can cause paranoia. As for a "disease", a physical one, well there are a couple I can think of...one would be a brain tumor, it causes changes in personalit/mood and paranoia, and another would AIDS-dementia complex, but that's all I can think of.

Paranoid Schizophrenia is one---Post Traumatic Stress Syndrome is another. I would go with PTSD as a starter. It can be triggered by any one or many events in your life.

A development of paranoia could turn into psychosis, which is kinda like thinking everything around you isn't real.
