A simple "thank you" is sufficient. Receiving birthday cards doesn't warrant sending out thank-you notes, if that is what you are asking.

Rule of thumb, if they don't have anything in the card ie. money, no thank you note is required. A simple thank you will do fo just a card.

Yes. Always say thank you, however if it's a gift then of course a handwritten thank you would be nice.

You could, but it would be impersonal and rude. Even if you just send them a store bought card, it will be much more appreciated.
