It was tagged on with the ayats.

Muhammad not only delivered the ayats, be he also delivered the arrangement of the ayats and what is the title of a collection of ayats.

The title of the surat refer to its content.

surah of the cow if I'm not mistaken is the surah of Al-Baqarah, in the surah there told a story about a cow that given to Moses and his people.. i think the title is selected to show the main point of the Surah..

i think the story went like this, a female cow was given to Moses and his people.. through Moses, God warned them not to kill the cow.. Moses' people was given food and drink from heaven and living lavishly but they disobey and kill the cow..

(I'm sorry that all i can remember, my teacher told me long time ago.. if you want full story, read the Al-Quran.. )

i believe the prophet (the only true filter of the koranic verses) named them..not sure. The prophet was given the gift of being a true filter for the koran. Abu Bakir al saddiq ordered them i believe..again not sure..

Allah named them Himself. Everything in the Koran is from Allah.

all i know is that it was named by god , it was sent to the prophet(peace be upon him) through the angel "jibril", the Quran was checked and reviewed by the prophet(pbuh), b4 his death and since then it was perserved unchanged,
