If he's constantly scratching, I'd have the vet check him for mites of an infection. For regular cleaning, if he's got a lot of hair in his ears, you need to pluck some of it. It doesn't all need to be cleaned out, just enough so air can circulate in the ear freely. If you don't know how, get a vet tech to show you. To clean his ears, all you need is warm water on a Q-tip. Be very gentle, don't go any deeper than you can see and he should be fine.

Go to the local chemist and, purchase a bottle of 3% medicinal peroxide. Combine peroxide and water 50/50 using a q-tip gently clean in all the ear creases and ear canal. Your dog may shake his, or her, head due to the bubbling of the peroxide, this is normal. Wash your pets ears with this same solution, rinse with fresh warm water, you should see a marked improvement in a couple of days. Do this every other day until problem clears up, and then once per week to prevent a recurrance. Hope this helps with the problem The odor in the ears is caused by bacteria that lives off the accumulated oils and dirt.

clean daily with one part vinegar and three parts water dry thoughly...also catch his ears hair only and band with a elastic band on top of his head for a few hours each day or at least when he is drinking or eating let the air get to the ears..he wont like it much at first but perservere as it helps a lot i have shihtzu and can have the same problem long hairy ears are a magnet for dirt and they get very warm...by letting the air get to them it helps a lot...alternatively you can get earwash from the vet but i found the vinegar solution is better i just wipe ears inside and the leathers daily with the solution and cotton wool and dry

You need to get a vet to check for mites as you won't be able to see them.If he is scratching at them there is a problem and cleaning won't help. The dogs coat shouldn't be getting matted ever as you should be grooming him daily. Take him to a groomer and get him properly trimmed and as for his ears to be trimmed for ventilation.

have just had this problem wiv my Lakeland Terrier. You will have to get some medication from the Vet, in my case it
was for 5 days treatment. then clean the ears twice a week
with a lotion you can get from any pet store (ie Pets at home)
the problem should be solved

We peg ours up every now & then to let them air, We also use a Specialist Ear Wipe from local vet. We trim the hair too, but no matter what the problem persists. keep at it though.

You need to pluck the hair inside the ears, if you don't want to do this yourself have your Groomer do it for you.
Also ears need regular cleaning with drops or powder as you can't see all the way down the ear channel.

Even if you don't see mites you should take him in to get checked anyway. Especially since you say you keep up on the cleaning and maintenance. He's probably uncomfortable.
