Uh...depends on the accent. If it's a hot accent, like British, Australian, or other European accents, then in my mind it goes something like " Hm...this is hot...i like it." For other not so hot accent, in my mind " Hm...must concentrate to understand." And usually I just thought about the accent and what country it's from...and whether I find it cute or not. Shallow I guess. But that's all I can think about.

Depends on what kind of an accent. I have an accent myself. I do not think I have ever been discriminated against.
What is important is not whether you have an accent or not (Brooklyners, Bostonions, Texans, Southerners, all have accents!), but how well you speak English.
I myself get impatient when someone has an accent (whether foreign or native american) that is hard to understand or the person speaks slowly (we americans like to do things efficiently and quickly).

This is totally dependant on the accent that I'm hearing
I am usually thinking of the placement of the individual to a geographic locale-- kind of like a rapid game of -- where is this person from ??---then the mind does the little momentary dance of images logged away of that particular place -- all of this unsually takes place in a zip zap moment-- then I go on to the next whatever----

I always wonder where they are from and what language they speak as their native tongue. I'm pretty good at figuring it out. Even dialects within Spanish and English. I can tell what country a Spanish-speaker is from and almost to a certainty what state an English-speaker is from. USA that is. Not with Brits.

I try to figure out where the person is from. Like a sort of challenge.
Usually I end up asking if I can't figure it out.
