You will need the damaged passport plus your original proof of citizenship ( i.e. state certified birth certificate or naturalization certificate - the damaged passport is not acceptable as proof of citizenship) plus a statement explaining what happened to the passport. Otherwise same procedure, time frame and fees as a first time applicant. quesitons - contact the passport center at 1-877-487-2778

If you are outside the country of issue for your passport, then check with your nearest embassy or consulate. Beware, some airlines and ports of entry don't accept passports that have been damaged - it looks suspiciously like a forged document.

If you are still in the country of issue, you'll need to contact the authorities that issued your passport.

Apply for a duplicate passport. You will get a new passport with new number.
