If a doctor becomes involved in a relationship with a patient in my state. The doctor can be fined and lose his license to practice. Before you do anything that would entice him into a relationship switch doctors.

How you could let him know about your feelings is ask him to refer you to another doctor. Tell him why you want to see another doctor. Also, mention that you've heard he could get into difficulty with the state licensing board if in fact he became involved in a personal relationship with a patient and the licensing board found out via an anonymous tip.

Most of us get a crush on someone who appears gentle, loving, helpful, etc. That's why teachers are sometimes put in similar situations. It doesn't matter what the rules are for a doctor/patient relationship...it is wrong and the doctor could lose his license and everything connected to it. If he is stupid enough to encourage you, you both will suffer in the long run; you, because you won't trust him (he has other patients, you know, who could also be bedding him) and him as stated above with licensing laws.
Change doctors, if necessary, or focus on other things when with him. He probably suspects your feelings, but if not, decide if he is a good enough doctor for you to continue this patient relationship. If he is, delay your gratification with someone else. Get active, get involved, but don't get in trouble.

Doctors love healthy people. If you're sick, I don't think he will ever get near you with all the virus that you produce

If there's a chance that he is interested in you, too, then you need to switch doctors.
