Sugar is a natural bacteriostat - ie things dont grow on it, it simply dehydrates living organisms.
Kept in a sealed jar it will last practically indefinitely, although the granules will stick together over time.

That depends on what the humidity of the brown sugar is at the time of packing into the bottle. If fully dried -this can be improved by assing de-watered compressed air though the brown sugar before botteling. The the shelf-life is indefinate, greater that ~ 5 years.
The shelflife "failure" is due to the moisture causing inversion of the sugar, this increases the humectancy of the sugar & every time you open to the air it can absorb more moisture (especially if you breath into it), eventually it all sticks together. This however will not apply to winter in a cold climate where you heat up the room then the humidity can drop to under 10%, but does apply to hot stick summers when the humidity is over 80%. - Like we are experiening in Durban - 30'C with a dew point of 24'C at present.

I don't think that it ever goes bad.
It can go hard but just put a piece of bread in the jar and an apple slice and it should soften up.
