Why not use flowers and decorate the top of the cake with them, or if there's one of those "paint your own pottery" stores in your area see if they have something that wouldn't be too heavy to set on your cake, you can paint it, if they have something make it something that you and your spouse to be or attendants paint.

I am unclear, you want to have it made for you or you want to do it yourself?
Florists will do custom cake tops. If you are having your cake custom made, then the designer can make a gumpaste topper for you that is custom made to your specs. If you have talent then try a decorating school, art school, do it yourself centre or do it at home.
