When the sugar first starts to go hard, adding a slice of apple to the bag and leaving it in the fridge for a few days will soften it up.
Once it has reached the 'rock hard' stage, you can soften it in the microwave. 30 seconds per cup of sugar, but make sure that it doesnt begin to melt.

get a mug of water and boil it in the microwave, so that there is steam. Put the opened bag of brown sugar next to it on the counter. cover it all under a larger pot or bowl and give it about 10 minutes. The steam will soften the brown sugar.

Wow. Please do not take any of the advice about heating it up or putting in microwave, you'll be sorry.
The ones about bread are right on but you should do that as soon as you open a bag of brown sugar, always keep a slice of bread or at least a half of a slice in the bag, and seal the bag tight.
Your best bet right now is to pour it in a bowl and squish the clumps in your hands, work them out yourself.
