I hope you dont some Marijuana but ill give you the benefit of the doubt. Eat foods rich in omega 3 fatty acids like wild salmon and trout but go with salmon really helps the brain. Play video games gamers what their called have better 3dimensional and visual memory. That's the ability to look at things in a room then leave the room and explain what items were in the room by detail.
Video games improve you memory. Play with some flash cards
or get a book on improving memory. Nutrition is huge avoid saturated fats they litterally clog you brain over time your 16 so i dont think thats a problem dont smoke weed that hurts your short term memory by 50% compared to non smokers.

I think it is like brain overload. Meaning if your under stress in your personal or school life, the mind take a break and this is the way your body protects itself. If possible, I would take a break from some of the stress of your daily life. Perhaps, walk in the park and yoga classes. Anything that would relax you to the point that your regain a sense of control.

Either you're just not concentrating, or you need to kind of loosen up. You may be under pressure, and not able to really think clearly. If you kinda cut down, and it's still happening, talk to your parents, and go see a doctor. Good luck!
