After doing 4 years of medicine in medical school. Some people decided to specialize further say surgeon or neurosurgeon. what's the situation? is it strict hard? impossible? just like medical school? easier? any comment would be helpful,

after med school, all you get is the MD degree. you cannot practice yet. therefore, every med school graduate has to enter a residency program. this is where they get trained in a certain area, whether it's family medicine or surgery or whatever else. these can take from 3-7 years. afterwards, you can practice on your own.

i think what you're referring to is people who continue into a sub-specialty, like thoracic surgery instead of just basic surgery. this adds on a few more years to your basic residency. i can't say whether one area is harder than another, but it is definitely harder to get into the appealing programs. for instance, dermatology is an easy lifestyle that pays well. it would be very competitive to get into a dermatology residency program. something like family medicine is a lot easier to get into.
