Did he actually apply for a card with all those fees? I've never even heard of the 2nd and 3rd. The 4th sounds like another way of saying the same thing as the first. If he is responsible for the fees, I'd say he had better have learned a very expensive lesson.
That said, read the contract. Any answer you get here that doesn't quote law is just a guess. Some that claim to quote law are as well.

I believe once you enrol, you still have to pay all the fees.
If you return it within 14 days, there may be a clause to get you out however you would have to check with the company.
