I would call up the company ASAP...sounds like identity theft...if he didn't open up the account and there's a credit card in his name with a bill...not good.

Credit card companies don't just send you the credit card ready to use. You have to apply for it.

Yes he can, but you need to look at the bill and ensure that it wasn't a processing fee. If it was, he'll still owe that amount. If it's an activation fee and he doesn't activate the card, he shouldn't owe that amount.
