I don't even want to get up and answer the door right now. Is it ok to just say "not at this time, I just can't"?

Yes tell your friend that you don't feel up to company but thank him anyway for his concern. Also, put a note on the door to say "Please do not disturb...thanks". If you have an answering machine you could also change the message to tell that you aren't up to talking with them right now but please leave a message.

Definitely. Most people (especially ones who have had an invasive surgery) would understand. You sound like you know how to phrase it nicely. Just tell them that you need your rest, but you would love a visit possibly later on.

yes it is fine. If it is your best friend they should understand. I don't even like people to visit me in the hospital the first day or so after surgery.

Of course it's ok to tell your best friend that you really don't feel like having any company and if she does't understand that - then she is very selfish.

Absolutely. You wouldn't be your charming self wincing in pain.

It isn't being selfish....you need time to heal.
